CSN builds individualized memory care plans for each person. Caregivers are prepared around the specific needs of the client, not generic dementia rules. They learn what helps with bathing, meals, transitions, activities, evening routines, reassurance, and safety.

Our team focuses on preserving dignity, reducing distress, maintaining familiar routines, and keeping the person safe while giving the family a clear plan. When something changes, sleep, appetite, mobility, behavior, toileting, or safety, the plan is updated and the family stays informed.

For ongoing dementia cases, nurse oversight helps connect daily caregiver observations to the bigger care picture so families have structure, communication, and a team that understands the disease.
